INSTRUCTIONS FOR REQUESTING DOCUMENTS OR LOG DATA IN WRITING:
Print out the patient information or log data request form on our website at hammashohde.fi/en or request a form from a Hohde clinic. Fill in the form and bring it to any Hohde clinic. When you bring in the form, you must identify yourself by presenting an identity document with a photo. This allows us to verify your identity and make sure that we are processing the correct individual’s personal data. We will send the information requested to your home address by post in 1–4 weeks.
ACCEPTABLE DOCUMENTS FOR IDENTITY VERIFICATION:
- – passport (issued by an authority of a Member State of the European Economic Area (EEA), Switzerland or San Marino)
- – identity card with a photo (identity card acceptable as a travel document, identity card of a minor, foreigner’s identity card, or temporary identity card)
- – driving licence (issued by a Finnish authority).
ACCESSING YOUR PERSONAL DATA
You have the right to access the patient information and other personal data pertaining to you. If you wish, you can request for this information to be sent on paper to your home address by post. See below for further information and instructions for requesting your personal data.
ACCESSING LOG DATA
You have the right to access the log data on the processing of patient information. The log data shows who has processed the information and when and for what purpose. You can request the log data in writing with the form available below or with encrypted email. Fill in the form and bring it to any Hohde clinic. When you bring in the form, you must identify yourself by presenting an identity document with a photo. This allows us to verify your identity and make sure that we are processing the correct individual’s personal data. We will send the information requested to your home address by post in 1–4 weeks. The log data for the national Patient Data Repository is available in the kanta.fi online service.
REQUEST A CLARIFICATION OR REPORT A SECURITY INCIDENT
If you suspect that patient information has been misused, you can ask us to look into the matter by submitting a free-form request for clarification in writing. The request must include your personal identity code as well as the date or another incident to which the request for clarification pertains. If you detect an information security incident that requires immediate or urgent actions, primarily contact the unit or personnel that treated you. As for other non-urgent matters related to information security, you can contact us through the feedback channel.
RECTIFICATION OF INCORRECT PERSONAL DATA
The customer has the right to request rectification of their incorrect personal data. With regard to personal data in OmaHohde, you can rectify your personal data yourself through your OmaHohde profile. For other personal data and patient information, you can find more detailed instructions below. For other personal data and patient information, you can contact the personnel who treated you and request rectification of your incorrect personal data.
Any corrections to patient information are done in such a way that the original entry can still be accessed if necessary.
ERASURE OF PERSONAL DATA
Based on the General Data Protection Regulation, the data subject has the right to be forgotten, i.e. request erasure of their personal data. This right to erasure applies to personal data that we process based on a customer relationship or consent provided by the data subject. For patient information, the right to erasure does not exist, as patient information is subject to the statutory obligation of health care units to retain the patient information created through their operations.
RIGHT TO OBJECT TO OR RESTRICT THE PROCESSING OF PERSONAL DATA
The data subject has the right to object to the processing of their personal data in so far as the personal data is processed based on Hohde’s legitimate interests, such as a customer relationship. With regard to patient information, the processing of personal data cannot be stopped, as patient information is subject to the statutory obligation of health care units to process and archive patient information. The data subject has the right to object to the use of their personal data for direct marketing purposes.
RIGHT TO LODGE A COMPLAINT WITH THE SUPERVISORY AUTHORITY
If the data subject deems that Hohde has violated data protection regulations, the data subject has the right to lodge a complaint with the Office of the Data Protection Ombudsman, which serves as the supervisory authority.
